Hours: Full time – (37.5 hours per week – may include evening or weekend work) Pay: Intensive Outreach Support Worker: £25,643 per annum (£13.15 per hour) Location: Westminster What does Support Work involve? As an Intensive Outreach Support Worker, you will work within the Community Engagement team to help people with learning disabilities to achieve greater levels of independence and resilience to keep safe, be healthy and live well. You will manage a case load of referred individuals with a maximum 25 hours of support. You will jointly complete an Outcome Star at the point of referral to help identify the levels of confidence each referral has in aspects of daily living. This will help inform the activities needed to address the purpose of the referral as well of offer a holistic approach to success. People want to be part of the community and to develop independence skills and so understanding the principles of doing with and not for is important. Therefore, you must be willing to work flexibly around the specific needs of your cases. This may include evening or weekend work where the outcome requires this support. You need to be digitally confident in use of Apps and other systems which help people be more independent and tracking progress. You will be part of a team and working as partners with professionals and families and so your communication skills and level of professionalism must be at the highest level. Being a team player who can take on additional responsibilities based on a key working model is also a requirement. Training will be provided.
