Thames Reach runs projects and services that help to end homelessness in London. We work directly with homeless people on the streets and in hostels. We work to prevent vulnerable people from becoming homeless. We help formerly homeless people get their lives back on track. About this role…
We are looking for a Lead Worker to join the City Outreach team.
Thames Reach’s Outreach teams operate across London and help those who are rough sleeping to find routes away from the street. The teams respond to referrals and work flexibly to reach London’s most vulnerable. Individuals are assessed to identify their support needs and support plans are created, including work to obtain identification, access health services and refer to other specialist agencies, including those with immigration issues.
You will:
- Be the first response to rough sleeping referrals conducting regular outreach shifts, with volunteers.
- Assess the support needs of newly verified rough sleepers in a person-centred manner.
- Manage a caseload of clients, identifying the quickest and most appropriate route off the street, and devise plans to achieve this.
- Work alongside partner agencies to ensure that clients are accessing appropriate support to access and sustain accommodation.
- Represent the team at multi-agency meetings, such as Task & Targeting meetings.
- Accurately record all work done.
To succeed as the Lead Worker, you will have:
- Experience in flexible and responsive services to people with a history of rough sleeping and homelessness.
- Strong communication skills with the ability to form and maintain effective working relationships with all partners.
- A commitment to supporting people to achieve the best outcomes as they work towards independence.
- Strong organisation skills, with the ability to manage conflicting priorities and deadlines.
- Experience to work autonomously as well as part a team.
- Strong IT skills with the ability use Outlook, online recording systems, Excel, and Word, using a tablet and a smart phone.
