About this role… We are looking for Lead Worker to join the Community Living and Support Service team. The Lambeth Living Well Network Alliance supports people in Lambeth who are experiencing mental illness or distress. Thames Reach works in an alliance with South London and Maudsley NHS Foundation Trust, Lambeth NHS Clinical Commissioning Group, Lambeth Council and Certitude to deliver mental health services as part of the innovative transformation of mental health services in the borough. You will: - Oversee transitions of care into the community for patients - Support the mental health system to improve practice in managing crisis - Work with inpatient wards to ensure discharges are safe and timely. - A strong understanding of the issues faced by individuals who suffer from complex mental health needs. - Ability to work with an MDT (multidisciplinary team) as well as in partnership with various stakeholders. - Strong advocacy skills to support those unable to advocate for themselves To succeed as a Lead Worker, you will have - Organised and efficient. - Excellent at communicating and problem solving - Able to effectively motivate and support clients - Competent at using a computer and a range of software packages - Committed to developing your knowledge and skills - Responsive and flexible to the needs of the team and the client We are listed in the top 100 UK Best Workplaces 2022 in the UK.
