Thames Reach runs projects and services that help to end homelessness in London. We work directly with homeless people on the streets and in hostels. We work to prevent vulnerable people from becoming homeless. We help formerly homeless people get their lives back on track. We help people find work and access training and volunteering opportunities.

About this role…
We are looking for a Lead Worker to join the Ealing Move-on team.
Thames Reach’s Supported Housing is comprised of Accommodation, Housing First, Navigator and Move on services supporting people to make real and lasting change in their lives. People may present with complex mental health, offending histories, drug and alcohol needs.
Move On services: Provide holistic floating support to clients at varying stages of their homelessness journey, including: people rough sleeping histories or people in temporary accommodation. Helping them navigate and access systems and services with the ultimate aim to secure accommodation in the community and prevent future homelessness.
As the successful Move-on Lead worker you will:
Provide advice and guidance to support workers
Source move on accommodation and identify new landlords
Work with rough sleepers who have no recourse to public funds, helping them resolve their immigration issues and identify the best route into accommodation and conduct person-centred assessments and create move on plans from these
Provide tenancy sustainment support, linking people into other local tenancy sustainment teams for ongoing post resettlement support where appropriate.
Work closely with the Ealing council services and commissioners to share information and best practice.
Ensure all records, incidents as well outcome are up to date on the rough sleepers database
To succeed as the Move-on Lead worker you will:
Have excellent prioritising skills and be able to independently manage a complex caseload
Be able to support and advise less experienced colleagues
Be a proficient user of Microsoft Office applications, including databases
Show initiative and attention to detail
Effective at developing and maintaining good and effective working relationships
Have experience of working with people with a rough sleeping history and be aware of the options available to them.
