About us… Thames Reach runs projects and services that help to end homelessness in London. We work directly with homeless people on the streets and in hostels. We work to prevent vulnerable people from becoming homeless. We help formerly homeless people get their lives back on track. About this role… We are looking for a Lead Worker to join the Ealing Move-On team. Thames Reach’s Supported Housing is comprised of Accommodation, Housing First, Navigator and Move on services supporting people to make real and lasting change in their lives. People may present with complex mental health, offending histories, drug and alcohol needs. Navigator and Move On services: Provide holistic floating support to clients at different stages of their homelessness journey helping them navigate and access systems and services with the ultimate aim of securing accommodation in the community and prevent future homelessness. You will: .Support people in TA with positive decision from home office. .Assess people with a range of support needs and identify a suitable Move-On Plan. .Be confident in working autonomously within the community. .Attend internal and external meetings representing the team / organisation .Be creative and proactive in problem solving. .Embrace the Whole team approach, working with colleagues and partners to reduce risks, support people to meet their goals and build aspirations. To succeed as a Lead Worker, you will have: .Excellent communication skills. .An empathetic and trauma-informed approach. .Experience in working in collaboration, with people in the services, your team, internal and external organisations. .Skills to ensure accurate data collection, recording and maintenance of an up-to date case management system. .Volunteer supervision experience, .Experience of working effectively with people that have a range of support needs using a person centred, outcome based approach.
