About the Role
The Mental Health Pathway Manager role is part of the Living Well Network Alliance (LWNA), a unique partnership delivering integrated Mental Health services alongside key alliance partners including South London and Maudsley NHS Foundation Trust, Certitude, Thames Reach, and South London ICB. This role offers a rewarding opportunity to coordinate and facilitate service user moves into, across, and out of mental health services, working closely with Providers, Mental Health operational health and social care staff, Housing, and Commissioning teams.
The postholder will manage flow within the pathway, including identifying and managing voids, tracking service user progress towards move-on, and addressing barriers and provider issues by finding collaborative solutions. A strong focus on information management is essential to support reporting and strategic decision-making across the pathway.

Experience
Experience in mental health social care or related service coordination is highly desirable. Proven ability to work collaboratively with multiple stakeholders and manage complex pathways effectively.

Qualifications
Relevant professional qualifications in social work or related fields are preferred. Candidates should demonstrate knowledge of mental health services and pathways.
