Outreach Caseworker Location: Hillingdon, Uxbridge UB8 3NN Hours: 35 per week Contract: Fixed Term for 6 months Salary: £28,821 per annum based on 35 hours per week plus Inner London weighting of £3,344, if applicable Could you help people who have complex medical / social needs? We are looking for an Outreach Caseworker to join our team of experts based in Hillingdon and be a key part of our national network. A day in the life of an Outreach Caseworker will include: You'll work in the community to help identify and address unmet social needs that may be exacerbating physical and/or mental health. You'll help people with complex presentations including mental health, substance abuse, multiple long-term conditions and adverse traumatic experiences. They're often suspicious of formality and offers of support, and your compassionate, authentic nature will help them access the help they need. You'll help to lower stigma within the community and change mindsets and behaviours. You'll achieve this by building strong relationships with partners and stakeholders, such as A&E departments, Community Mental Health Teams, GP surgeries, housing teams etc. To be a successful Outreach Caseworker: Do you have experience in person centred support planning? Can you identify risks and anticipate issues? Are you able to make a difference to people's lives, by putting in place interventions that reintegrate people into the local community? This would be done without the need to make demands on services that may not be the most appropriate to meet their needs. Are you resilient and motivated? Can you demonstrate compassion, strong negotiation skills, patience and a real appetite for helping some of the most vulnerable people in our society?
