About the Role
Join our dedicated team as a Prison Leaver Recovery Practitioner, playing a vital role in improving continuity of care, reducing reoffending, and empowering individuals on their recovery journey. You will support men and women both pre- and post-release, building strong relationships with prison substance misuse teams and community services to ensure seamless transitions and sustained engagement with drug and alcohol support. The role is based within Via Harrow, an established Community Integrated service that is well performing and highly functioning. The service is CQC registered and provides a range of clinical interventions including BBV and health interventions, OST, community detox, alongside robust family, carers, and service user involvement provisions.
Experience
Experience working with individuals affected by substance misuse or within criminal justice settings is desirable.

Qualifications
