About The Role

The Assurance, Strategy and Improvement team are looking for an experienced programme manager to join the team to support the development and implementation of the integrated health and care portfolio of work in Islington.
Our health and care services are experiencing significant challenges driven by an ageing population and rising prevalence of people living with long term, complex conditions and needs. Life expectancy is stalling, and key risk factors are driving a significant and unequal burden of preventable ill health and premature death, further widening health inequalities among our population.
There is widespread consensus nationally among health and social care professionals, the NHS and government policymakers that the changing needs of the population require changes to the way health and social care services are organised and delivered. The prevalent view is that by not moving toward a whole system working and planning care services around the needs of the individual, systems will continue to experience gaps in provision, duplication in effort, wasted resources and poor population outcomes.
The aim of the integrated care programme in Islington is to address the above-mentioned challenges by developing an embedding innovative service models that can offer holistic, person-centred and coordinated approach to delivering health and care to our population.
By embedding these new ways of working we hope to provide our residents with a more seamless access to services, offering timely advice and support and an enhanced quality and experience of care that enables them to live healthy independent lives for as long as possible in their own homes, or the place they call home.
The programme manager with work closely with the Programme Director for Integration, and will have the following key responsibilities:
- To ensure the programme has a clear and proportionate programme management infrastructure including suitable governance and reporting arrangements and resources.
- To identify risks, issues and dependencies within the programme and with other programme and projects.
- To ensure that the programme is supported by a clear and effective communication plan directed to all relevant stakeholders.
- To ensure the programme is coherent and consistent with the successful delivery of departmental and corporate strategy.
- To work in a matrix setting with internal and external stakeholders who have specific responsibilities to support projects or work- streams within the Integrated Care Plan.
We are looking for someone with experience of multi-professional and/or inter-agency working and understanding of the challenges that integrated working can bring, as well as knowledge of innovative new models of care in adult social care and health and effective change implementation.
