The Role Senior practitioners hold a smaller caseload of some of the service's most complex and vulnerable service users. Their complexities will include physical and mental health, homelessness and insecure housing, domestic violence and abuse, and isolation. Case management will involve providing regular, focussed support and advocacy, risk management, engaging and liaising with other services to provide wrap-around care, and assertively and creatively trying to re-engage individuals who are disengaging. Senior practitioners will support early-career practitioners to develop their skills and knowledge and perform additional activities to support team leaders. A senior practitioner role is an opportunity to get involved in and learn more about the strategic, operational and management activities of substance misuse treatment, helping you develop your portfolio of expertise and experience for future career advancement. We are seeking committed, organised and highly motivated candidates, with an in-depth knowledge and experience of the challenges affecting people with substance misuse and related health and wellbeing issues. The Service Launched in 2018, Via Merton is a fully integrated substance misuse service, providing clinical and psychosocial interventions, operating from a main treatment hub, with additional satellites within the community. The team actively promotes recovery and works with individuals that are directly and indirectly affected by substance misuse, working with the local community to improve the health, wellbeing and functioning of Merton residents, irrespective of circumstance.
