About this role…
We are looking for a Support Worker to join the Hammersmith & Fulham Outreach and Housing First team. The team forms a significant part of the boroughs complement of services working with rough sleepers, working to reduce the number of people rough sleeping in the borough, by providing short sharp interventions to support people to access accommodation and ensure that the appropriate support is in place. The team also works with a cohort of housing first clients, providing intensive floating support to people to access / maintain a tenancy and work towards recovery and integration.
Thames Reach’s Outreach teams operate across London and help those who are rough sleeping to find routes away from the street. The teams respond to referrals and work flexibly to reach London’s most vulnerable. Individuals are assessed to identify their support needs and support plans are created, including work to obtain identification, access health services and refer to other specialist agencies, including those with immigration issues.
Thames Reach welcomes applications from people with transferable skills.
You will be responsible for:
- A small caseload of housing first clients, offering support to access and maintain accommodation.
- Managing a caseload of rough sleepers, identifying the quickest and most appropriate route off the street, and devising a plan to achieve this, while holding a caseload of your own.
- Conducting regular outreach shifts, with volunteers and partner agencies.
- Representing the team at multi-agency meetings such as Task & Targeting meetings.
- Identifying gaps in provision and researching new pathways off of the streets for a variety of rough sleepers, including those with no recourse to public funds.
- Accurately recording all work done
To succeed as the Support Worker, you will have:
- Have excellent interpersonal skills
- Be flexible and responsive to meet the needs of rough sleepers
- Have the confidence and ability to manage a caseload of complex clients
- Have the necessary IT skills to collect, collate and interpret data
- Be confident in representing Thames Reach at external meetings and with outside agencies
