Salary: £30,859 per annum
Hours: 37.5 per week
Contract: Fixed term until 12/2024
Location: London – Lewisham, Southwark and Tower Hamlets

Thames Reach runs projects and services that help to end homelessness in London. We work directly with homeless people on the streets and in hostels. We work to prevent vulnerable people from becoming homeless. We help formerly homeless people get their lives back on track. We help people find work and access training and volunteering opportunities.

About this role…
Thames Reach’s hostels are transitional spaces where a 24 hour staff team support people to move in, assess their needs, and quickly identify the best place for them to move on to next. Skilled staff teams develop personalised support plans to ensure people are ready to move on and are accessing support with their health, substance misuse, criminal justice and life skills needs.
We are looking for a Support Worker to join the Resettlement Support Team .
Thames Reach welcomes applications from people with transferable skills.
You will:
- Work alongside the Lead Worker and Senior Practitioner supporting vulnerable adults to move on from hostels to appropriate accommodation.
- Work with multiple partner agencies including accommodation providers, local outreach teams, and various specialised support teams
- Use a flexible approach to supporting a caseload with a wide range of support needs and barriers to accessing services
- Regularly update notes and outcomes on our database system.
- Represent Thames Reach and clients during communication with external partners and local authorities.
To succeed as the Support Worker you will have:
- A strong commitment to providing a high standard of service
- A willingness to support adults with a varying range of needs
- Excellent organisation and time management skills to remain strongly outcome focused
- Good communication skills, both verbal and written
- Experience in forming strong working relationships with partner agencies
- Strong IT skills with the ability to use Outlook, online recording systems, Excel, and Word, using a tablet and a smart phone.
