About this role… Floating Support services work with people to sustain their housing. We use a strength-based approach to work with people to identify what support they require and how we can address them together or / and in collaboration with others e.g. landlords, Department of Work and pension or mental health services. We can support people for the duration of their tenancy, or we can provide short term interventions. We can provide resettlement support when a tenancy commences, and we can equip people with the skills to manage their home and goals independently or when they are move on ready and require minimal support. You will: Visit and meet people in their own homes and across different community locations in London Support people to stay safe and well and to achieve their goals and aspirations Work in partnership with housing providers, and other services, to help people to manage their home and avoid a return to homelessness Help people to manage their health and wellbeing and to be connected with their local community Support people to develop the skills and confidence to move on and keep their own permanent home Work collaboratively with people we support, and partner services, to help develop and enhance the service that we offer to people To succeed as the Support Worker, you will have: An empathetic and effective outlook to support people with different needs Experience to be reflective and open to learning, to develop and enhance our support to people Experience to be flexible, adaptive and responsive in meeting the needs of people we support Experience in helping people articulate their needs, views and aspirations to you and to others To promote, and positively represent, Thames Reach through your work and through meetings and events with partner services Experience in organising and managing your time to meet the needs of the people we support We are listed in the top 100 UK Best Workplaces 2023 in the UK.
