About the Role
The 16-25 Link Worker role is designed to provide vital support and guidance to young people aged 16 to 25, helping them navigate health, social, and educational challenges. You will work closely with individuals to connect them with appropriate services and resources, promoting positive outcomes and wellbeing. This role requires strong communication skills, empathy, and the ability to work collaboratively within a multidisciplinary team. You will be an essential link between young people and the services they need, contributing to their personal development and community integration.
Experience
Experience working with young people aged 16-25 in a health, social care, or community setting is desirable. Previous involvement in case management, advocacy, or youth support roles will be advantageous.

You are compassionate, patient, and motivated to make a positive difference in young people’s lives. You possess excellent interpersonal and communication skills, are able to build trust quickly, and work effectively both independently and as part of a team. You have a strong understanding of the challenges faced by young people and are committed to promoting equality and inclusion.
Qualifications
A relevant qualification in health, social care, youth work, or a related field is preferred but not essential. A willingness to undertake further training and professional development will be supported.
