About the Role
Join the London Borough of Sutton's Adult Social Care team as a Social Worker within one of the four Locality teams aligned to the Primary Care Network footprints of Wallington, Carshalton, Sutton & Cheam. This role focuses on supporting older people, individuals with physical and sensory disabilities, and those with mental health needs through integrated, multidisciplinary working alongside NHS colleagues. Your key responsibilities will include undertaking Care Act 2014 assessments and reviews, managing safeguarding enquiries, conducting Mental Capacity assessments, participating in duty rota, DSTS and CHC processes, and court-related work.
We are seeking experienced Social Workers with a passion for person-centred care and excellent communication skills to join a supportive, energetic team dedicated to delivering outstanding care plans that prioritise service users' needs and voices. This role offers a Welcome Bonus of £1000 paid in the first month or a Retention Bonus of £1000 for every complete year of service (subject to payscale).

Experience
- At least 2 years post ASYE working experience as a UK registered social worker
- Experience working in a multidisciplinary team setting
- Experience undertaking complex mental health and physical health cases
- Experience managing safeguarding enquiries and conducting Mental Capacity assessments

- Compassionate and able to manage high levels of emotional distress
- Excellent communication skills with the ability to clearly explain complex social issues to patients and professionals
- Ability to plan and coordinate social care plans and aspects of patient discharge
- Enthusiastic about delivering person-centred care and working collaboratively within a team
Qualifications
- Registered with Social Work England
- Degree in Social Care or a related field
