The Assessment and Review Worker role is based within a very busy Integrated Discharge Team at Whiston Hospital. Working closely with the Trust, the team ensures safe and timely discharges for patients and their families. This fast-paced environment requires individuals with the skills and motivation to join an enthusiastic multi-disciplinary team focused on safe and timely patient discharge across multiple local authorities.

Recently, the team joined the multi-disciplinary Contact Cares Team, offering additional career development opportunities. The work is diverse, challenging, and rewarding, requiring the ability to work independently while contributing to a larger team. The team is supportive, with a mix of skills and experience.

Requirements

Experience
Experience working in a fast-paced, multi-disciplinary environment is desirable. Ability to work independently and as part of a team is essential.

Qualifications
Relevant qualifications or experience in health, social care, or related fields are advantageous. Enhanced DBS Disclosure required.
