Location
Westminster, with hybrid working arrangements including on-site at local practices, South Westminster Centre for Health (nearest tube Pimlico), or Capital House on Chapel Street (nearest tube Edgware Road).
Hours
37.5 hours per week. The service aims to run 7 days a week (excluding Bank Holidays), requiring availability for evening and weekend work.

Healthcare Central London Ltd (HCL) is a federation of 27 General Practices and 4 Primary Care Networks in Westminster, serving over 275,000 patients. We provide community and primary care services alongside management and administrative support to member practices. Our unique community-led care model, known as ‘The Octopus’, supports integrated neighborhood teams and tackles inequalities in healthcare.
The Care Coordinator role is pivotal within the Adults ADHD service (Right to Choose), providing referral management, appointment booking, and care coordination. You will work closely with healthcare professionals and service users to ensure smooth navigation through care pathways, maintaining accurate records and supporting clinicians. This role requires excellent communication, administrative skills, and the ability to work both autonomously and as part of a team.
The position involves managing service inboxes, booking appointments, maintaining clinician rotas, handling queries sensitively, and contributing to service development. You will engage with multiple stakeholders including NHS GP Practices, local authorities, and partner organizations to deliver high-quality care and support.
Experience
- At least 1 year of experience in a customer service-oriented environment
- Experience working in Primary Care
- Excellent skills using SystmOne clinical system

- Understanding of personalisation, public health agendas, and Digital First Primary Care programme is desirable
- Commitment to participate in training programmes as required
