About the Role
The Care Coordinator will work closely with PCN practices and the multidisciplinary team to provide a central coordination role in patient care planning and delivery, primarily within the Network's Home Health Programme. The role involves coordinating care packages for patients and carers as identified by clinicians across health, social care, and mental health services. You will act as a single point of access for staff and service users, supporting continuity of care and serving as a contact for families, residents, and professionals for patients on your caseload.
Key responsibilities include actively managing patient care plan delivery, liaising with clinicians and allied professionals, assisting patients with queries, appointments, and access to self-management information and interventions. You will support PCN activities around immunisation, including the Covid-19 vaccination programme, and contribute to the development of new services such as the Home Health Programme.
Additional duties include supporting data collection and reporting for dashboards, KPIs, and performance targets, coordinating and delivering multidisciplinary team meetings including minute taking, maintaining accurate patient records, and assisting with audit information. You will also support call and recall programmes for vaccinations, patient participation groups, and help patients use surgery pods and other online services.
The role requires adherence to Bromley by Bow Health Partnership policies on health and safety, infection control, confidentiality, data protection, risk management, equality and diversity, and safeguarding vulnerable adults and children. You will be part of a collaborative team dedicated to delivering high-quality healthcare and tackling wider determinants of ill-health while maintaining the integrity and working practices of member practices.
Performance management, personal development, and training are actively encouraged and supported to ensure continuous professional growth.
Experience
Experience working within a healthcare or multidisciplinary team environment is desirable. Experience in care coordination, patient liaison, or related roles will be advantageous.
About you
You will be organized, proactive, and able to manage multiple priorities effectively. Strong communication and interpersonal skills are essential to liaise with patients, families, clinicians, and external agencies. You should be committed to delivering high-quality patient care and able to work collaboratively within a team. A positive attitude towards learning and adapting to new ways of working, including remote working, is important.
Qualifications
Relevant qualifications or experience in health or social care are preferred but not mandatory. You must be willing to undertake all required statutory and mandatory training and comply with continuing professional development requirements.
