About the Role
As a Care Coordinator within the Primary Care Network (PCN), you will work closely with PCN practices and the multidisciplinary team to provide a central coordination role in patient care planning and delivery, primarily within the Network's Home Health Programme. You will coordinate care packages for patients and their carers as identified by clinicians across health, social care, and mental health services. Acting as a single point of access for staff and service users, you will support continuity of care and be a key contact for families, residents, and professionals for patients on your caseload.

Your role includes actively managing patient care plan delivery, liaising with clinicians and allied professionals, assisting patients in managing their needs through answering queries, managing appointments, and providing quality information to support informed care choices. You will also help patients access self-management resources and interventions that promote health and wellbeing.

Additional responsibilities include supporting PCN activities around immunisation programmes, including Covid-19 vaccinations, assisting in the development of new services such as the Home Health Programme, coordinating data collection and reporting for performance monitoring, supporting multidisciplinary team meetings including minute taking, and maintaining accurate patient records.

You will contribute to initiatives that tackle wider determinants of ill-health, work collaboratively within a team to maximise benefits to patients and the community, and support new ways of working including remote working. You will also support call and recall programmes for vaccinations, assist Patient Participation Groups, and help patients use surgery pods and other online services.

We are committed to delivering high-quality, equitable healthcare with low bureaucracy, fostering a partnership based on mutual trust, respect, and shared vision.
