About the Role
The PCN Care Coordinator plays a vital role in supporting the Primary Care Network (PCN) by managing patient call/recall, appointment booking, and proactive reminders via texts, telephone, and letters. You will work closely with the PCN to identify and manage patient caseloads, supporting personalised care and acting as a key liaison for patients, carers, relatives, and health and social care professionals. Your responsibilities include coordinating and navigating patients across health and care services alongside other PCN staff and primary care roles, supporting enhanced services and other PCN requirements, and contributing to tackling inequalities in health and social care, particularly for patients with long-term conditions.

You will run searches and reports using the clinical system EMIS Web, support the coordination and administrative functions of multidisciplinary teams (MDTs), and provide ad hoc support to the PCN team and clinical leads. Administrative duties include managing DOCMAN, patient registrations, and preparing reports, spreadsheets, and presentations as required.

Communication is a key part of this role, providing face-to-face support to patients at PCN practices and community venues, responding to telephone and email enquiries, and assisting patients in managing their care needs. You will follow up with patients who do not attend appointments and communicate effectively with patients and carers, adapting communication methods to meet diverse needs. Accurate record keeping and coding in patient medical records is essential.

Additionally, you will develop and maintain effective working relationships with internal and external stakeholders, liaise with outside agencies and community centres to coordinate care pathways, manage mail merges and bulk texts for campaigns such as flu vaccinations and cancer screening, and obtain and evaluate patient feedback.

Quality improvement is also a focus, including reviewing data accuracy in clinical systems, supporting data reconciliation, performing data quality checks, and correcting errors.

Requirements

Experience
Experience working within a healthcare or primary care setting is desirable. Familiarity with clinical systems such as EMIS Web and managing patient communications is advantageous.

Qualifications
Relevant qualifications or experience in healthcare administration, social care, or a related field are preferred but not essential. A willingness to learn and develop within the role is important.
