About the Role
Central London Healthcare CiC is the GP Federation representing 30 General Practices across the Westminster area, supporting 4 Primary Care Networks (PCNs). We are seeking a Care Navigator on a fixed term contract until 31 March 2027 to provide care coordination for the most vulnerable and complex patients. The role involves supporting GPs by managing intelligent tasking and patient referrals, completing Care Plans, and delivering targeted public health messages. You will work collaboratively across health, social care, and third sector organisations to ensure coordinated, patient-centred care and effective delivery of Care Plans for patients identified through risk stratification.
The role is predominantly non-patient facing, with support mainly provided via telephone. You will be aligned to specific GP Practices within the GP Federation and work across several practices within the designated PCN structure. There is also an opportunity to assist with the Complex Case Management service, offering targeted support in specialised areas.

Experience
- Experience using SystmOne
- Knowledge of Medical Terminology
- Good knowledge of the Health & Social Care system
- Strategic understanding and experience of working in the NHS, especially Primary Care Services
- Experience creating Personalised Care & Support Plans
- Ability to build trust and rapport and structure conversations using a coaching approach
- Flexible and adaptable to changing needs and working times including covering annual leave and sickness

Qualifications
- GCSE passes at Grade C or above or equivalent experience
- Educated to degree level in a relevant subject
- Excellent standard of written and verbal English and numeracy
