About the Role
Central London Healthcare CiC is the GP Federation representing 30 General Practices across the Westminster area, supporting 4 Primary Care Networks (PCNs). We deliver a range of NHS contracts including Community Dermatology, Cardiology, Secondary Care Referral Services, and out-of-hospital care. As a Care Navigator, you will play a vital role in supporting GPs by coordinating care for the most vulnerable and complex patients. This includes intelligent tasking, patient referrals, completing Care Plans, and delivering targeted public health messages. You will work collaboratively with colleagues across health, social care, and the third sector to ensure patient-centred, coordinated care. The role is predominantly non-patient facing, with support mainly provided via telephone, and involves working across several GP practices within the designated PCN structure.
Your responsibilities will include using clinical systems such as SystmOne to maintain accurate patient records, responding to tasks promptly, supporting patients via telephony and email, and maintaining knowledge of local third sector services. You will chair and participate in multidisciplinary team meetings, provide navigation across health and care services, and proactively identify additional support needs. The role requires flexibility, collaboration with the Care Navigator team, and a commitment to delivering timely and effective care coordination. Experience
- Experience using SystmOne
- Knowledge of Medical Terminology
- Good knowledge of the Health & Social Care system
- Strategic understanding and experience working in the NHS, especially Primary Care Services
- Experience creating Personalised Care & Support Plans
- Ability to build trust and rapport and structure conversations using a coaching approach
- Flexible and adaptable to changing needs and working hours including cover for annual leave and sickness

Qualifications
- GCSE passes at Grade C or above or equivalent experience
- Educated to degree level in a relevant subject
