About the Role
The Care Navigator will provide World Class Care to service users, staff, colleagues, clients, and patients at the Royal Free London NHS Foundation Trust. The role involves facilitating access to local services by receiving and processing referrals from GPs and multidisciplinary teams, assessing patient needs, and signposting to appropriate health, social care, voluntary sector, and community services within the borough. The post holder will support individuals to access services that promote health, wellbeing, independence, and social inclusion, including funded and self-funded options. Identifying and assisting unpaid carers to access relevant support is a key responsibility. The role also includes collaborating with volunteers, providing general information on benefit entitlements, and developing personalised support plans shared with GPs, carers, and other professionals. Maintaining up-to-date knowledge of local services and promoting awareness among healthcare professionals is essential.

Additionally, the Care Navigator will support personalisation by assisting individuals to access Community Care and Carers' Assessments, monitoring progress, and liaising with local authorities to ensure timely support.
