About the Role
Join Medacs Healthcare as a Community Nurse (Band 5) and provide exceptional nursing care to patients within the Brentford community. This temporary role offers flexible working options, allowing you to work your way while making a meaningful difference. You will deliver high-quality, person-centred care, supporting patients to maintain their independence. Key responsibilities include assessing, planning, implementing, and evaluating care plans, collaborating with multidisciplinary teams, GP surgeries, and acute trusts, and assisting with hospital discharges to reduce readmissions.

Experience
A minimum of 6 months NHS experience as a Community Nurse is required.
