As Head of Service – Community Wellbeing & Resilience, you will lead the development of integrated community wellbeing services, creating stronger connections between health, social care, communities and the voluntary sector. You will have responsibility for the hospital social work team, reablement, discharge to assess, occupational therapy and the multi-agency joint emergency team.
Your leadership will focus on breaking down barriers between organisations and services, strengthening prevention, reablement and early intervention, reducing avoidable escalation of need and unnecessary admissions, improving access to coordinated support, and enabling communities and voluntary organisations to play a greater role in promoting wellbeing and resilience.

Experience
Proven experience of leading transformational change and delivering measurable improvements in services and outcomes.
Strong track record of partnership working across complex systems including health, social care, housing, local government or community organisations.
Experience of delivering services through collaboration, influence and partnership working.
