About the Role
The Leaving Care Team is seeking a motivated, skilled, flexible, creative, and innovative Leaving Care Worker to join our team. This full-time role involves working with young people aged 16 to 25 years old, supporting their transition into adulthood by helping them gain essential independent living skills. You will engage with young people in semi-independence provisions, residential settings, and those placed at home, working closely with Horizons Education Centre, LAC teachers, nurses, and psychologists to ensure positive outcomes.

As part of Children’s Services, all care leavers are offered opportunities to develop skills needed for effective transition into adulthood. The successful applicant will manage a caseload of approximately 25-26 young people, which will involve travel across the country.

The Leaving Care Team has a strong reputation for positive results, with many young people successfully obtaining degrees, apprenticeships, and training opportunities.

Requirements

Experience
Experience working with young people aged 16-25, particularly care leavers, is highly desirable. Knowledge and understanding of the emotional wellbeing of care leavers and the ability to support their development of independent living skills is essential.

About you
You will be motivated, flexible, creative, and innovative with strong interpersonal skills. You should be committed to working within a values-driven practice model and able to build positive, trusting relationships with young people and colleagues. The ability to manage a caseload and travel as required is necessary.

Qualifications
Relevant qualifications in social work, youth work, or a related field are preferred. You must have the right to work in the UK, with evidence requested at interview. The post holder will be subject to an enhanced Disclosure and Barring Service (DBS) check and Ealing’s enhanced vetting process.
