About the Role
As a Liver Surveillance Navigator at Royal Free London NHS Foundation Trust, you will work as part of a dedicated team within a speciality patient group to coordinate all administrative functions from GP referral through to patient discharge. Your role is pivotal in ensuring the smooth running of patient pathways, facilitating timely and efficient care delivery. You will assist patients with multiple specialist needs by coordinating their pathways to reduce the number of hospital appointments across the Trust.
You will be responsible for managing patient referrals, maintaining accurate patient records on Cerner, and ensuring all administrative processes align with clinical pathway instructions and Trust policies. This includes contacting patients to provide instructions and reminders, arranging transport and translation services, and supporting satellite and off-site locations.
In addition, you will monitor clinic workflows, reschedule and book follow-up appointments in partnership with the Outpatient Appointment Centre (OAC), and escalate any pathway delays or bottlenecks to the clinical pathway manager. You will also ensure all appointments comply with the Trust Access Policy and national targets.
Providing World Class Care is central to this role, ensuring patients, families, and colleagues feel welcome, respected, confident, and reassured throughout their care journey. You will handle confidential patient-sensitive documentation, respond to concerns or complaints appropriately, and support data quality through audits and validation.
Key responsibilities include liaising with clinicians to place patients on correct pathways, ensuring pre-investigations are ordered, notifying relevant departments of patient deaths to cancel appointments, and overseeing the transfer of referral information for patients referred to other trusts.
Experience
Experience in patient administration or healthcare coordination is desirable. Familiarity with clinical pathways and NHS administrative systems such as Cerner is advantageous.

Qualifications
Relevant administrative or healthcare qualifications are beneficial but not essential. A commitment to continuous professional development and adherence to Trust policies and procedures is required.
