About the Role
The Mental Health Pathway Manager role is part of the Living Well Network Alliance (LWNA), a unique partnership delivering integrated Mental Health services with key partners including South London and Maudsley NHS Foundation Trust, Certitude, Thames Reach and South London ICB. This role offers the opportunity to work within a collaborative alliance model focused on coordinated mental health service delivery.

The postholder will be responsible for screening, coordinating, and facilitating moves into, across, and out of mental health services. This includes managing flow through the pathway, identifying and managing voids, tracking service user progress towards move-on, and addressing barriers in partnership with Providers and relevant stakeholders. A strong emphasis is placed on information management for reporting and supporting strategic decision-making.

Experience
Experience in mental health social care or related service coordination is desirable. Proven ability to manage complex pathways and work collaboratively with multiple partners is essential.

Qualifications
Relevant social work or health and social care qualifications are preferred. Candidates should demonstrate knowledge of mental health legislation and pathway management.
