About the Role
The Mental Health Pathway Manager role is part of the Living Well Network Alliance (LWNA), a unique partnership delivering integrated Mental Health services with key partners including South London and Maudsley NHS Foundation Trust, Certitude, Thames Reach and South London ICB. This role offers a rewarding opportunity to coordinate and manage the flow of service users through mental health pathways, working closely with providers, operational health and social care staff, housing, and commissioning teams. Responsibilities include screening, coordinating, and facilitating moves into, across, and out of services, managing voids, tracking service user progress towards move-on, and resolving barriers in partnership with providers. A strong focus on information management and reporting supports strategic decision-making within the pathway. Experience
Experience in mental health social care or related service coordination is highly desirable. Proven ability to work collaboratively with multiple partners and manage complex pathways is essential.

Qualifications
Relevant professional qualifications in social work, mental health, or related fields are preferred. Candidates should demonstrate knowledge of mental health services and pathway management.
