You will act as the designated Disability Champion on the ward, leading the multi-disciplinary team in Disability Equality matters within the service and other areas as required. Your role will promote recovery and social inclusion, contributing to the care pathway from admission through to discharge in line with team principles. You will plan for robust, sustainable discharge and aftercare, involving clients and their families in planning and evaluating care. Additionally, you will contribute to individual care plans from an occupational therapy perspective, maintain clinical records, participate in the acute care forum, develop and evaluate the ward's activity timetable, and supervise junior staff such as the activity coordinator.
