About the Role
As a Patient Care Coordinator at Pastel Health, you will be the compassionate first point of contact for all patient enquiries via telephone and email. You will manage the onboarding process for new patients, welcoming them to the service and explaining the next steps clearly. Your role includes responding promptly to incoming queries, providing clear advice and support to patients throughout their journey.

You will coordinate patient pathways by managing new referrals and monitoring patient progress to ensure a seamless journey from referral to discharge. Leading appointment scheduling for both remote and in-person clinics, you will align bookings with clinician availability and efficiently handle rescheduling or cancellations.

Maintaining accurate patient records within the Electronic Patient Record (EPR) system is essential, ensuring data quality and strict adherence to confidentiality and GDPR regulations.

Additionally, you will provide comprehensive administrative support for Multidisciplinary Team (MDT) meetings and Clinical Operations meetings. Managing written and verbal correspondence with GPs and other healthcare professionals, including clinic letters and information requests, will be part of your responsibilities.

You will actively capture feedback from patients, their families, and carers regarding their experience with Pastel Health. Handling initial patient complaints or concerns with empathy and tact, you will escalate issues when necessary and support service improvement initiatives to help continuously enhance the quality of care delivered.

Experience
Experience in patient communication, healthcare administration, or a related field is desirable but not mandatory. Strong organisational skills and the ability to manage multiple tasks efficiently are essential.

You are compassionate, patient-focused, and an excellent communicator both verbally and in writing. You have a proactive approach to problem-solving and can handle sensitive situations with empathy and professionalism. You are comfortable working within a multidisciplinary team and maintaining confidentiality at all times.

Qualifications
A relevant qualification in healthcare administration or customer service is advantageous but not required. A willingness to learn and adapt within a healthcare environment is essential.
