About the Role
The Patient Flow Co-ordinator plays a vital role in supporting the coordination of safe and timely patient flow across the hospital, with a strong focus on effective discharge planning. Working closely with the multidisciplinary team (MDT), the post holder will help reduce delays, support patient-centred discharges, and ensure the efficient use of hospital beds. Key responsibilities include coordinating safe, timely, and effective patient discharges in line with Trust policies, promoting early discharge planning from admission, and attending daily ward board rounds and MDT meetings to monitor patient progress and prioritise discharges. The role involves tracking planned discharge dates, facilitating same-day discharges and transfers to community or rehabilitation settings, and escalating any delays promptly to senior staff. The co-ordinator will work collaboratively with nurses, doctors, therapists, social workers, and discharge teams to support coordinated care planning and maintain effective relationships across wards, departments, and external agencies. Acting as a key point of contact for discharge-related queries, the role requires clear and sensitive communication with patients, families, and carers about discharge plans and timelines. Additionally, the post holder will liaise with community services, social services, care agencies, and other healthcare providers to organise care packages and placements, support patient transport arrangements, and ensure discharge logistics are completed efficiently. Administrative duties include maintaining accurate patient information on electronic systems, recording discharge plans and delays, supporting discharge documentation completion, and assisting in data collection for service monitoring. The role also involves identifying inefficiencies in patient flow, supporting service improvements and discharge initiatives, participating in audits, and promoting a proactive discharge culture. The co-ordinator must work within their scope of competence, adhere to Trust policies including safeguarding and information governance, maintain patient confidentiality, and contribute to a safe and effective working environment. Flexibility is required to meet requests that support patient discharge from internal and external stakeholders as circumstances change.

Requirements

Experience
Experience in a healthcare or hospital environment is desirable, particularly in discharge coordination, patient flow, or multidisciplinary team working. Proven ability to work collaboratively with clinical and non-clinical teams to support patient-centred care is essential.

You will be organised, proactive, and able to manage multiple priorities in a fast-paced environment. Strong communication skills are required to liaise sensitively with patients, families, carers, and healthcare professionals. You must be flexible and adaptable to changing circumstances and able to escalate issues appropriately. A commitment to maintaining confidentiality and adhering to policies is essential. You should be motivated to contribute to service improvements and work effectively within a multidisciplinary team.

Qualifications
There are no specific formal qualifications required; however, relevant healthcare or administrative qualifications would be advantageous. Mandatory training and ongoing professional development will be supported by the Trust.
