About the Role
As a Patient Pathway Coordinator within the Neonates service, you will play a vital role in ensuring smooth and efficient patient care pathways. You will communicate clearly, effectively, and compassionately with the multidisciplinary team, patients, their families, visitors, and carers, reflecting the Trust’s values. Your responsibilities include managing telephone and email enquiries, acting as a point of contact for the department, and resolving simple queries or escalating complex ones appropriately.
You will coordinate patient care by monitoring and updating PTL to ensure RTT pathways are accurate and compliant with Access Policy. You will proactively work with clinicians and managers to resolve delays and escalate breaches as necessary. Booking patient appointments, diagnostics, inpatient stays, and managing patient correspondence via text reminders and letters will be key tasks.
Additional duties include managing clinical staff inboxes, virtual clinics, and internal correspondence, supporting rota management, and attending meetings as required. You will also be responsible for accurate data entry and management within the Epic system, ensuring patient demographics are correct, supporting patient registration on the MyChart App, and contributing to data quality initiatives.
This role requires a high level of organisation, communication skills, and the ability to work collaboratively with clinical teams to deliver excellent patient care.
Experience
Experience in patient administration, coordination, or a healthcare setting is desirable. Familiarity with clinical systems such as Epic is advantageous.

Qualifications
