About the Role
We are seeking a dedicated PCN Care Coordinator / Healthcare Assistant to join the North Lambeth Primary Care Network team at Mawbey Group Practice. This permanent role involves undertaking delegated clinical tasks and duties following clear protocols, supporting patients and carers, and working closely with the wider clinical team. You will communicate effectively with patients, carers, and team members, attend PCN clinical coordinator meetings, and assist in promoting health and well-being within the practice.
Key responsibilities include maintaining the nursing notice board, providing health information leaflets, using problem resolution skills to manage patient concerns, and escalating issues to senior clinical colleagues when necessary. You will accurately record patient information, maintain confidentiality, and contribute to clinical governance by adhering to agreed standards of care.
The role also involves supporting service development and quality improvement initiatives, managing clinical stock control alongside the nursing team, and staying informed of current patient care approaches. You will participate in professional development activities, including supervision and appraisal, and assist with clinical audits.
Health, safety, and security are paramount; you will identify and manage risks related to your work, adopt infection control procedures, safely manage clinical equipment, and assist patients and colleagues in maintaining a safe environment.
General duties include supporting the smooth running of the practice and participating in housekeeping and domestic needs as required.
Experience
Experience in a healthcare or clinical support role is desirable but not explicitly stated. Ability to undertake delegated clinical tasks and follow protocols is essential.

Qualifications
