About the Role
As a Care Coordinator within the Waltham Forest Federated GP Network, you will facilitate and ensure the effective delivery of patient-centred, personalised health and social care plans. You will monitor progress, report outcomes, and contribute to patient reviews and care planning within appropriate time frames. You will explain patient pathway management to clinical staff, liaising between services and service users, and contact services using appropriate procedures and referral mechanisms. Working closely with all relevant care agencies including primary care, secondary care, community services, Mental Health, Social Services, Ambulance Service, Voluntary services, and other providers, you will ensure coordinated care plans without requiring further GP referrals. You will maintain accurate records and statistical returns as required by the CCG, including entering patient-related information into Clinical Reporting Systems within required time frames. Ensuring proper handover of care between different settings, you will manage mutual transfer of communications and patient notes and ensure care packages are set up. You will collect data on patients and carers for recognised outcome measures and update patient notes to reflect any changes, including care plans. You will manage operational meeting processes by identifying patients for discussion, working closely with clinicians to define and lead meetings, organising and attending relevant meetings including Integrated Care meetings, and ensuring all necessary documentation is circulated in advance. You will ensure meeting actions are recorded, disseminated, and followed up promptly, keeping relevant practitioners informed and chasing action resolution. Networking and developing strong relationships with NHS key local players including the CCG, GPs, Social Services, Mental Health Trusts, Community Trusts, and voluntary sector providers will be essential. You will also present in MDT meetings with GPs and Nurses and act as a contact point for GPs and practices, establishing systems and processes to ensure timely and appropriate responses to queries from clinicians and other stakeholders.

Requirements

Experience
Experience working within health or social care coordination or a similar role is desirable to effectively liaise with multiple agencies and manage patient care pathways.

You have excellent communication and interpersonal skills to build strong relationships with clinical staff, patients, carers, and external agencies. You are able to work independently and as part of a multidisciplinary team. You demonstrate attention to detail in record keeping and data management. You are committed to delivering high-quality, patient-centred care and able to handle sensitive information with confidentiality.

Qualifications
Relevant qualifications in health or social care or equivalent experience are preferred. Training and development opportunities will be provided to support your role.
