About the Role
As a Pharmacy Technician within Modality Partnership, you will play a key role in supporting medicines optimisation and safe prescribing across the Partnership. Your responsibilities will include medicines reconciliation following hospital discharge and other care transitions, supporting repeat prescribing processes and medication reviews, and assisting with structured medication reviews and high-risk drug monitoring within your competency. You will respond to pharmacy-related patient enquiries and support medicines advice services, complete prescribing audits, and contribute to quality improvement initiatives.
Working collaboratively with Clinical Pharmacists, GPs, prescribing teams, and pharmacy administrators, you will maintain accurate clinical records and ensure compliance with prescribing policies and procedures. You will also use digital systems and innovative technologies to improve patient access and prescribing efficiency, contributing to the ongoing development of the Pharmacy Hub and new models of care.

Experience
Experience in pharmacy services, particularly in medicines optimisation, repeat prescribing, medication reviews, and high-risk drug monitoring is desirable. Experience working within primary care or community pharmacy settings will be advantageous.

Qualifications
Mandatory vaccinations: Hepatitis B vaccination history, MMR, and Varicella vaccinations are required for this role. Proof of UK working eligibility is required at interview. Two references are required, including one from your current or most recent employer, covering at least the past three years of employment or training.
