About the Role
The primary purpose of this role is to take blood samples and blood pressure readings from patients known to have, or at risk of, long term health conditions. You will complete computer 'check & test' templates and contact patients to book appointments for reviews of long term conditions, screening, immunisation, or other required services. Maintaining accurate records of call and recall activity is essential. You will offer general assistance to the Practice team and patients, projecting a positive and friendly image both in person and via telephone.

You will receive, assist, and direct patients to the appropriate service or healthcare professional efficiently and courteously. Facilitating effective communication between patients, primary care team members, secondary care, and other healthcare agencies is a key responsibility.

Duties include identifying patients needing vaccination, immunisation, health advice, screening, or review using clinical records; contacting patients by telephone, text, or letter; providing information on immunisations and screening, including addressing misinformation; updating patient contact details; and booking appointments.

You will also be responsible for maintaining and monitoring the Practice appointments system alongside the reception team, processing appointment requests via personal, telephone, and online channels, and ensuring patients are directed appropriately. A working knowledge of telephone systems and an appropriate telephone manner are required.

Additional responsibilities include computer data entry, processing and recording information according to Practice procedures, working flexibly to meet Practice needs, undertaking mandatory and additional training, and performing any other duties appropriate to the role as directed by the Practice Manager or Lead Clinician.

Confidentiality is paramount; you will handle sensitive patient and Practice information with discretion and in accordance with policies and legislation. You will also assist in promoting health, safety, and security within the workplace, including infection control and reporting hazards promptly.

Personal and professional development is encouraged through participation in training programmes, annual performance reviews, and attendance at meetings and update courses.

Maintaining quality within the Practice is essential, including alerting team members to quality and risk issues, assessing own performance, contributing to team effectiveness, and managing time and resources efficiently.

Effective communication within the team and with patients and carers is vital, including recognizing and responding to alternative communication needs.

You will also contribute to the implementation of Practice policies, standards, and guidelines and participate in audits as appropriate.

Requirements

Experience
Experience in phlebotomy or care coordination is desirable but not explicitly stated. The role requires competency in clinical record systems, telephone communication, and appointment management.
