About the Role
The Primary Care Health Care Assistant will work closely with the clinical lead and service manager to develop and implement physical health checks for the Severe Mental Illness (SMI) pathway. Additionally, the post holder will collaborate with the GP based at Haringey Advisory Group on Alcohol (HAGA) and the service manager to perform comprehensive health checks for patients with substance misuse. This is a patient-facing role focused on supporting and improving the overall health outcomes of patients within the community.
Experience
Experience working in a healthcare or community setting is desirable but not essential. The role requires the ability to engage with patients and support clinical pathways effectively.
