About the Role
As a Primary Care Mental Health Recovery Worker, you will provide medicine education, adherence advice, and lifestyle guidance to patients and their carers presenting with mental health problems. You will support early intervention and shared decision-making around self-management. Working collaboratively with patients and team members, you will encourage engagement with appropriate services and treatments, including onward referral to secondary mental health services or signposting to other mental health services, social prescribers, voluntary services, and community-based support groups.
You will support patients transitioning from secondary care to primary care by providing up to three step-down sessions at the point of discharge from secondary care services, as well as assist patients moving from primary care to secondary care when more specialist or intensive support is required. Your role will also involve helping patients maintain wellness within their local communities, reinforcing newly learned psychological techniques.
In addition, you will contribute to quality improvement initiatives aimed at enhancing physical health outcomes for those with long-term mental health conditions and mental health outcomes for those with long-term physical health conditions, as agreed with your line manager.
You will provide mental health-specific training and shared learning for staff across the primary care network, including offering specialist advice related to mental health. Mandatory and statutory training as required by Trust policy will also be undertaken, with identification of individual training needs.

Experience
