About the Role
Southwark’s Reablement Service is seeking a passionate and dedicated Reablement Officer to support residents in achieving greater independence. This role offers the opportunity to work directly with people at important moments in their lives, helping them regain skills, overcome barriers and develop confidence to live as independently as possible. The service focuses on prevention, early intervention and strengths-based practice, supporting residents following illness, crisis, housing instability or significant life changes through personalised plans tailored to individual goals and abilities.

As part of Housing Needs and Support, you will contribute to improving outcomes for residents through prevention, tenancy sustainment and effective move-on support. You will collaborate with colleagues across housing, health and community services to ensure residents receive timely and appropriate support. What You'll Be Doing
As a Reablement Officer, you will provide practical, person-centred support to residents in their homes and communities. Your responsibilities will include:
- Working alongside residents to understand their circumstances, strengths and aspirations, creating personalised reablement plans
- Helping residents develop confidence, practical life skills and maintain independent living
- Supporting residents moving on from supported accommodation or facing tenancy risks
- Providing proactive, community-based support to prevent repeat homelessness and reduce long-term intervention needs
- Managing a caseload of residents with diverse needs, ensuring coordinated and reviewed support focused on positive outcomes
- Building effective partnerships with housing officers, landlords, housing associations, health professionals, voluntary organisations and community services
- Connecting residents with local resources, community networks and wider support to promote wellbeing and resilience
- Maintaining accurate records, monitoring progress and contributing to service improvement through effective practice and partnership working

You will be a visible and trusted presence within the community, helping residents overcome barriers, recognise strengths and achieve meaningful outcomes.

Requirements

Experience
Experience supporting individuals with diverse needs, particularly within housing, social care, community support or related services is essential. Demonstrated ability to build positive, trusting relationships and engage effectively with people from a wide range of backgrounds is required.

You are confident working independently, managing a caseload and responding effectively to changing priorities.
