About the Role
Following an assessment, you will provide practical and recovery-focused support to service users and carers as outlined in their care plans. Working collaboratively with care coordinators or lead clinicians, you will deliver clearly defined recovery-focused interventions in line with individual care packages. You will attend and actively participate in team meetings, care plan review meetings, and team training as appropriate. Your role will involve positively promoting and supporting independent living for service users within the community by maintaining extensive knowledge of and links with community resources, actively assisting service users to access these services. Building a professional rapport with service users based on support and collaboration, while maintaining appropriate boundaries, is essential. You will provide regular practical support to help service users maintain dignity and independence, enabling them to acquire daily living skills such as shopping and cooking. Additionally, you will assist service users in accessing benefits, welfare rights, health promotion information, and physical health services, including monitoring their health needs within the community. You will also help identify early signs of relapse by monitoring clients’ progress, level of functioning, and mental state, updating care coordinators or other involved staff accordingly.
