About the Role
Join the London Borough of Bexley Adult Social Care team supporting the Community Mental Health Team - Psychosis Pathway. We are transforming our practice model to provide the highest level of care, promoting dignity, choice, control, and independence for adults. The role supports Oxleas NHS Foundation Trust’s redesigned Intensive Case Management in Psychosis (ICMP) pathway, which focuses on assertive outreach, multidisciplinary support, and recovery-oriented care.
As a Social Care Assistant, you will work within a multi-disciplinary team to assess the needs of service users and their carers, developing creative and practical programmes to enhance quality of life. The role requires a firm grasp of Adult Safeguarding, knowledge of the Care Act, and a commitment to person-centred, recovery-focused practice.

Experience
Experience working in adult social care or mental health settings is desirable. A firm understanding of Adult Safeguarding and working within a multi-disciplinary environment is essential. Knowledge of the Care Act is preferred.

Qualifications
Relevant qualifications in social care or mental health are desirable but not mandatory. Appointment to this post is subject to an Enhanced Disclosure and Barring Service (DBS) check. Candidates must meet the English language requirements for public sector workers as set out in the job description and person specification.
