About the Role
A Social Prescriber Link Worker empowers people to take control of their health and wellbeing by providing time, focusing on what matters to the individual, and taking a holistic approach. The post holder connects people to community groups and statutory services for practical and emotional support. They support existing groups to be accessible and sustainable, help people start new community groups, and work collaboratively with all local partners.
The role is based within a specific Primary Care Network and Enhanced Access Service, working in partnership with practice teams to support patients and promote lifestyle changes. This includes preventing ill-health through behaviour change techniques.
The post requires a good understanding of social prescribing, wellbeing, community services, and the ability to provide motivational and behavioural support. Social prescribing strengthens community and personal resilience, reduces health inequalities by addressing wider determinants of health such as debt, poor housing, and physical inactivity, and increases active involvement in local communities.
This role particularly supports people with long-term conditions (including mental health), those who are lonely or isolated, or have complex social needs affecting their wellbeing. The salary reflects the complexity of cases and the significant level of multi-agency working, including supporting community groups to receive referrals.
Experience
Experience in social prescribing or related community health roles is desirable. Understanding of wellbeing, community services, and behaviour change techniques is essential.
