About the Role
As a key member of the Primary Care Network (PCN) multi-disciplinary team, you will promote social prescribing within local GP practices and the wider community. You will build and maintain strong relationships with staff across the PCN, attending relevant MDT meetings to provide information and feedback on social prescribing. Your role will involve proactively developing links with local agencies to encourage referrals and supporting efforts to raise awareness of social prescribing as a means to reduce pressure on statutory services, improve health outcomes, and enable a holistic approach to care. You will contribute to programmes that gather regular feedback on the quality and impact of social prescribing services and use systems and technology to encourage self-referrals and connect with all local communities, especially those that statutory agencies may find hard to reach.
Experience
Experience working within health, social care, or community settings is desirable. Proven ability to build and maintain effective relationships with a range of stakeholders including healthcare professionals and community organisations.

You will be proactive, confident, and able to work independently as well as part of a team. Strong communication and interpersonal skills are essential to engage with diverse communities and encourage referrals. You should be motivated by improving health outcomes and supporting people to access appropriate services.
Qualifications
Relevant qualifications or experience in health, social care, community development, or related fields are preferred but not essential. A commitment to ongoing professional development and learning is important.
