PO1 - PO4 £41,133 - £52,194 per annum
Retention Bonus: £2,000 retention bonus paid in £1,000 twice a year

About the Role
The team plays a vital role in enabling safe, timely discharges from hospital settings by working collaboratively within a multidisciplinary framework. Empowering residents with the tools, confidence, and support needed to transition back into their home environment—whatever form that may take or pathway that may be required.
At the heart of the team’s work is a commitment to maximising each resident’s opportunity to return home and live as independently as possible. By building on existing support networks and strengthening connections within local communities, the team helps residents develop the skills and resilience to be self-sufficient, while also ensuring appropriate safeguarding measures are in place.
Through this coordinated and person-centred approach, the team strives to support residents in living fulfilling, independent lives and achieving the best possible outcomes for their future.
We are seeking high calibre and creative applicants who are committed to building on residents’ strengths and their natural support networks, rather than simply assessing eligibility for services. Social workers in this role believe in giving residents as much choice and control as possible over their day-to-day lives, especially during recovery periods.
As a social worker, you will hold a varied caseload working with residents across discharge pathways. Responsibilities include completing Mental Capacity Act assessments, human rights assessments, NRTPF cases, supporting homeless residents, managing complex family dynamics, and safeguarding vulnerable adults.
Please note, this role does not offer Skilled Worker visa sponsorship; applicants must already hold UK work authorisation not requiring sponsorship.

Requirements

Experience
Previous experience working in adult social care or transferable skills relevant to this post are required.

About you
You are a high calibre, creative professional committed to a strength-based approach that empowers residents and supports their independence. You are confident managing complex cases, including safeguarding and mental capacity assessments, and able to work collaboratively within multidisciplinary teams.

Qualifications
Current registration with Social Work England is essential, and you must be able to provide your registration number.
The role requires satisfactory employment references, CIFAS identity checks, declaration of interest, DBS check, and continued professional body registration.
Fluent spoken English is required for effective performance of the role, with confidence and accuracy, but this does not refer to any specific English language qualifications or accents.
