About the Role
The Community Active Recovery Team (CART) within Adult Social Care supports residents new to social care services, focusing on preventative work to help vulnerable adults live independently. The team provides reablement support for up to 6 weeks, offering proactive case management in collaboration with occupational therapists, physiotherapists, community health colleagues, and equipment suppliers.
We seek high calibre, creative social workers committed to empowering residents by building on their strengths and natural support networks, promoting choice and control over their lives. Responsibilities include managing varied caseloads, conducting human rights assessments, and supporting complex cases requiring more intensive intervention.
The team includes Integrated Care Management (ICM) workstreams working in Multi-Disciplinary Teams (MDTs) with community health professionals to prevent hospital readmissions and institutional care. Additionally, the Continuing Health Care (CHC) workstream involves close collaboration with the Integrated Care Board to assess eligibility for CHC funding through MDT Decision Support Tool meetings.
Vacancies are available across all three CART workstreams. Please note this role does not offer Skilled Worker visa sponsorship; applicants must have existing UK work authorization.
Experience
- Previous experience in adult social care or transferable skills relevant to the role.
- Knowledge and experience of issues affecting people in need, including relevant legislation, guidance, and procedures.
- Experience delivering culturally sensitive services to a diverse community.
- Ability to work effectively as part of a team and under regular supervision.
- Ability to work under pressure, prioritise work and risks, and meet performance indicators.
- Experience and working knowledge of Strengths Based Practice.
- Detailed knowledge of the Care Act 2014 and other relevant legislation relating to adults and families.
- Awareness of current care and support policy and practice issues.
- Understanding of financial implications for Local Authority commissioning and alternative funding streams.

Qualifications
- Current registration with Social Work England with a valid registration number.
- Satisfactory DBS check and compliance with safer recruitment practices.
