Could you be the vital link helping people transition safely from hospital back into the community? Are you ready to use your social work expertise to make a meaningful difference when it truly matters? Join our Transfer of Care (TOC) Service, where you will support adults with care and support needs to achieve safe, timely, and effective hospital discharges. This is an exciting opportunity to work within a fast-paced, multidisciplinary environment, helping adults move smoothly from hospital or rehabilitation settings back into the community or onward care.

Key responsibilities include:
- Supporting safe and timely discharges from acute hospitals and rehabilitation settings
- Promoting independence, wellbeing, and person-centred outcomes
- Undertaking strengths-based assessments and care planning
- Managing complex cases involving physical disabilities, sensory impairments, dementia, and complex health needs
- Working collaboratively with health professionals, housing services, and voluntary sector partners

Requirements

Experience
- Registered Social Worker with Social Work England
- Strong foundation in adult social care and/or health services
- Experience of strengths-based assessments and outcome-focused care planning
- Confidence working within multidisciplinary teams
- Ability to manage complex cases and competing priorities
- Excellent understanding of safeguarding procedures and best practice
- Up-to-date knowledge of adult social care legislation, including the Care Act and Discharge to Assess (D2A) pathways

You are calm under pressure, reflective in your practice, and committed to person-centred care. You demonstrate a strong commitment to equality, diversity, and inclusive practice, with professional curiosity and dedication to maintaining high standards of social work practice.

Qualifications
Must be a qualified Social Worker registered with Social Work England. This post requires a satisfactory Disclosure and Barring Service (DBS) check at Enhanced - Adults and Children (Barred) level.
