About the Role
An exciting opportunity has arisen for a dedicated Support Coordinator to join the Community Learning Disability Team. The post holder will contribute to delivering the team’s objectives by providing effective case management and supporting adults with learning disabilities to achieve the best possible outcomes. Responsibilities include undertaking Care Act assessments, support planning, care plan reviews, safeguarding enquiries, and multi-disciplinary working. The role also involves participating in the team’s duty system.
The Community Learning Disability Team is a friendly, motivated and dedicated group of social care professionals working in line with the Care Act 2014, Mental Capacity Act 2005, and other relevant legislation. The team collaborates closely with a multi-disciplinary health team including Clinical Psychologists, Speech and Language Therapists, Community Nurses, Occupational Therapists, Physiotherapists, Counsellors and Behaviour Analysts to support adults with learning disabilities and complex needs. The team also manages transitions from Children’s Social Care and Special Education Needs services to adult social care in accordance with the Children and Families Act 2014.
We are passionate about working closely with residents and vulnerable groups to help maintain independence and balance support with independent living.

Experience
Experience working with adults with learning disabilities and an understanding of the challenges they and their families face is desirable.
