Location
Based within Royal Borough of Kensington and Chelsea (RBKC). Please note this service does not have step free access.
Hours
37.5 hours per week on a rota Monday to Friday, primarily working 09:00 to 17:00 with some shifts working 13:00 to 21:00. You may be required to work outside these hours as per service and resident requirements. This role also includes bank holiday working.
Salary
£28,800 per annum
About the Role
We are hiring a Support Mentor to join our team based in RBKC. In this role, you will support residents who have multiple and complex needs including homelessness, mental health, and substance use challenges.
As a Support Mentor, you will identify individual resident needs and provide person-centred, tailored support to help residents overcome their personal challenges and achieve their goals. These goals may involve sustainable living, independence, and overcoming mental health and substance use difficulties.
You will achieve this through producing tailored support plans, organising activities, and working in partnership with other agencies. You will support the continuation of trauma-informed practices and help create a psychologically informed environment that is safe, respectful, and responsive, putting people and communities at the forefront.

Key Responsibilities Include:
- Supporting residents facing multiple and complex needs
- Creating tailored support plans through one-to-one sessions, group sessions, and multi-agency meetings
- Helping residents build skills and confidence to move towards independent living and overcome personal challenges
- Monitoring residents’ wellbeing, carrying out regular welfare checks, and responding to incidents or emergencies
- Supporting the planning and delivery of activities to engage residents with others, communities, and opportunities, including days out and in-service activities
- Keeping support plans updated on online portals as appropriate
Experience
Previous professional or lived experience supporting people with multiple and complex needs is essential.

You are self-motivated and proactive in managing your time during busy or quiet periods to ensure productivity. You are comfortable using and adapting to IT at an intermediate level, including Microsoft Office and other software programs. You are flexible with working patterns to meet the needs of the service and residents.
You understand the social marginalisation often attached to people with multiple and complex needs and are committed to supporting them effectively.
