About this role… We are looking for a Support Worker to join the team at Hillingdon outreach. Thames Reach’s Outreach teams operate across London and help those who are rough sleeping to find routes away from the street. The teams respond to referrals and work flexibly to reach London’s most vulnerable. Individuals are assessed to identify their support needs and support plans are created, including work to obtain identification, access health services and refer to other specialist agencies, including those with immigration issues. Thames Reach welcomes applications from people with transferable skills. You will: - Directly casework people who are rough sleeping, supporting them to access appropriate services and accommodation. - Be a first response to referrals, carrying out assessments with new people met. - Carry out nightshifts in Heathrow airport. - Support lead workers to work with complex cases. - Work alongside other agencies including, the local authority, health services, housing providers and airport police. To succeed as the support worker ,you will have: - Excellent interpersonal skills - The ability to be flexible and responsive to meet the needs of people who are rough sleeping - The confidence and ability to manage a caseload of complex clients - The necessary IT skills to collect, collate and interpret data - The confidence to represent Thames Reach at external meetings and with outside agencies.
